Summoned in the bottom of the cellar in where the mobs for The Legend of Trengal Keep (Part 6) are. To summon, you need to click on the bird-cage looking thing near the altar while possessing Nishek's Bladed Whip.

To obtain Nishek's Bladed Whip, complete the quest Nishek's Summoning, offered by Revenant Toridra in one of the side rooms of the cellar.

When you trigger the event, NISHEK will enter the room from the hallway, spawning a pair of adds (level 30ish 4-dots) at each set of pillars in the room. NISHEK will not aggro while spawning the adds, so it is recommended to not attack NISHEK until she reaches the altar, first killing the adds as quickly as possible.
